✈️電子問診票を開始します✈️
2026年8月15日（土）より、初診・再診ともに電子問診票を開始します。
ご予約後、ご来院前に問診票をご入力いただけるようになります。
あらかじめご入力いただくことで、ご来院後の問診票記入のご負担を減らし、よりスムーズに診療へご案内できるようになります。
電子問診票は、当院の予約システム「ドクターキューブ」からご入力いただけます。
なお、電子問診票をご利用にならない場合は、これまでどおり紙の問診票をご利用いただけます。ホームページから紙の問診票をダウンロードし、あらかじめご記入いただくことも可能です。
皆さまのご理解とご協力をお願いいたします。

２０２６年８月の休診日は、以下の通りです。
・毎週木曜日、金曜日
・１１日（平日の祝日）
・２９日、３０日（第5土日は休診日です）
★お盆期間中も、上記休診日以外は土曜、日曜（８日、９日、１５日、１６日）を含め、全日開院いたします★
受診される際は、ホームページまたは LINE（再診の方）より、ご予約をお願いいたします。
